I asked my doctor this question (I have a prescription for Xenical). She said there's no data to suggest that there's any danger in taking orlistat for a year or even more. My doctor will continue to prescribe providing I have a 5% weightloss over each 3 month period until I reach a healthy BMI. I made my 3 month goal very easily and know I have reached my second 3 month goal which ends in a couple of weeks. So it looks like I will have at least 9 months on the medication.
